The Vision

Medicine isn't easy but that doesn’t mean the process needs to be difficult. The process can certainly be easier with the evolution of technology. In 2013 I started TCPA, a provider group now seeing over 15,000 encounters a month and with the sole purpose of bridging the gap between the hospital and the successful transition to a patients home. Far too often patients fall between the cracks of transition along the medical spectrum. The 90 y.o. with a hip fracture and heart failure is expected to follow up with her Orthopedic surgeon and primary care physician within one week of hospital discharge for pneumonia. No one has talked to her about the end of life. No one has even attempted to understand her struggle to get to her appointments. The result: The patient is too weak to follow up, feels unwell and ends back up in the hospital. This is a cost to the system and it may cost this patient their life with unnecessary pain and suffering. TCPA’s aim was to solve that problem. 

Resistance to change makes change difficult. From the get go this problem was apparent and eventually gave birth to SamePageMD. Automated workflow to off load facility nursing staff saves time in this environment of nursing shortages. Our patient engagement tools improve the nursing experience and allow families to be easily involved in care that is transparent and transformative. Families become an extra set of eyes to ensure safety and a direct transparent link to Providers and Nurses allow a birds eye view for both parties while pushing direct engagement. Having simple access to patient information shouldn't be hard and communicating the issues should be easy.
